Profiles
A client has two types of profiles: a public profile and a private profile. This article will cover how to access the client’s:
- Public Profile
- Assessments
- Notes
- Referrals
- Photo Upload
- View Permissions
- Private Profile
Public Profile
In a client’s public profile, information and modules related to the public profile may be public and shared to other agencies on the Outreach Grid Network.
Assessments
Public Profile Assessments are assessments made for a client in their public profile. This information is captured and shared with other agencies on the platform unless otherwise configured. For information about configuration, ask your project manager or review the video on Configurations.
Client Assessments may be taken at any time. Depending on the assessment, such as the Shelter Preference Assessment, custom tags and notifications may be generated for a client based on their answers to the questions. You may add additional assessments at any time.
Notes
In the Notes tab, you can record any known hazards that the client may have so that service providers from other agencies are aware of them when providing services. Comments are public to other users in the network as well and can be used to help better identify the client.
Referrals
Users can view the progress of referrals they’ve created or may have been tagged in.
You can also view the progress of your referrals in the Referrals tab on the Dashboard.
Upload Photo
For clients, you can upload their photo from your device's gallery or from your camera. Make sure to allow permissions to the app to use it by selecting ‘Yes’.
View Permissions
In the "View Permissions" tab, you can view the permission waivers that were signed by the client as well as all of the waivers that have not yet been signed.
If the clients would like to sign their outstanding waivers, tap on sign and check the permission waivers to sign.
Private Profile
In a client’s private profile, information and modules related to the private profile are shared only within your agency.
Assessments
Private Profile Assessments are assessments made for a client in their private profile. This information is captured and accessible by users in your agency on the platform.
It is highly recommended that you make assessments in real-time as you are working with the client.
Case Notes
Case Notes are private notes that are made on a client’s profile. These notes are accessible only to users within your agency.
You can log your case note as a phone call, text/email, in-person meeting, or case note. We can set up your environment to have different case note types as well, if requested.
Client Status Logs
In the Client Status Logs tab, you can track the history of the client’s status from first contact to housing as well as change their status.
