A Client’s Public Profile holds all of the information collected about a client from first contact to housing, and is available for all agency’s in your Outreach Grid network to view.
This article will give an overview of some of the tools available on the Public Profile:
Clicking into a Client’s Public Profile immediately displays the client’s name, AKA, gender, age, date of birth, ethnicity/race, last known location, how many users have viewed this client, the client’s permission, and any associated tags.
Timeline
Scroll down to see the client’s timeline. The “Timeline” logs all actions and encounters made by users in regard to the client on Outreach Grid’s platform. As users add referrals, record encounters, conduct assessments, and make shelter reservation requests with this client, the timeline is updated accordingly with the most recent action done shown at the top.
Under the timeline tab, there is an option to view the Client Resource Index and External Data Matches.
Select the “Client Resource Index” to view the calculated index based on how intensely the client required resources and attention of users in your Outreach Grid Network.
Click on External Data Matches to view possible incident matches to the client.
Locations
The “Locations” tab on the Public Profile logs all encounters with the client by all users on the platform. The map to the right gives a visualization of the areas and hotspots that the client may frequent.
Click on “Record Encounter” if you would like to record a new encounter with the client.

Notes
The “Notes” tab is where hazards and comments about the client can be logged. These notes are public and can be viewed by other users in your Outreach Grid Network.
Case Managers associated with the client can also be seen in this tab.
To log a potential hazard, click on the red “Add Hazard” button, and a module will appear. Hazards are customizable by jurisdiction.
For additional comments, tap on the freeform text box under “Add New Comments”.
Referrals
Referrals are requests for work or information made to other users or agencies on the Outreach Grid Network. When you make a referral, you are alerting selected users about work that needs to be followed up for a camp or client.
To make a new referral, type in a subject and description of the work and tag the appropriate individuals or groups you want to follow up on the work. Adding a due date is optional.
A notification and email will be sent to the individuals or groups you tagged, and as they progress in the work request, you will receive updates via notifications and emails.
Media
The “Media” tab is where media including images of the client and PDFs related to the client and their cases can be uploaded. To upload a file, either drag a file into the gray box under “Add Media,” or click the gray box and choose the appropriate file from your device’s library.
A thumbnail of the media file will then appear in the gray area with a status bar.
The upload is complete when the status bar is complete and a check mark appears.
Files Checklist
Any files required for the client's case management can be uploaded to the "Files Checklist" tab. Under this tab, you can quickly see the total number of files to be collected, and how many have been submitted so far.
For each filetype, if you have the file scanned or saved on your device, you can submit it by first clicking "Choose File" and locating and selecting the file from your library. Click the green "Submit File" button to then upload the file to the Client's Profile.
Households
The Public Profile also lists client's household information. If a client belongs to the same household as another client in the Outreach Grid platform, those clients can be associated, and the head of household can be set.
By clicking the green "Add Household Members" button under the "Household" tab, you will be guided to a page to manage the members of the client's household. A list of clients on the platform will appear and you can select the appropriate clients. You can also use the search bar to find the associated client. Please note that the household member must already be a client created in the Outreach Grid system before they can be associated to another client's household. After selecting the appropriate clients, click the green "Submit" button at the bottom of the page.
You will then be navigated to a page to assign specific relationships between the head of household and the selected associated clients. After selecting the appropriate relationship between the clients, you can complete the association by clicking the green "Associate Clients to Household" button.
After associating the clients to the household, you will be redirected to the head of household's client profile. The "Household" tab will now be updated on the client's page, as well as the "Household" tab for any other clients that were associated to that household.
Chat
In the “Chat” tab, outreach workers and service providers can exchange information about a client here in real-time.
