The survey begins by recording your current location. Follow the on screen prompts to set your location. If you are part of a team, you may notice that your team name is listed at the top of the screen, your team's area is highlighted, and that there are locations marked on the map. The following page prompts you to select a survey. Depending on the event, there may be multiple surveys, or there may be only one. Select “Start Survey” below the appropriate assessment to continue.

Completing the Survey and Subsurveys

Some surveys begin with screening questions. These questions ensure that you have the client's consent and also prevent the collection of duplicate data sets. The responses to these questions must be “Yes” in order to complete the survey.

After submitting the screening questions, you can complete the survey based on the answers of the respondent. After submitting the respondent’s survey, you can then collect information about that individual’s household.

The Outreach Contributors App collects household information using subsurveys. To begin, a sliding counter appears to indicate how many additional household members the respondent is reporting.

This number is used to create the appropriate number of subsurveys. As you complete the subsurveys, a counter will indicate the current subsurvey that you are completing.

After completing the survey and all of its paired subsurveys, you will be taken back to the events page, where you can begin a new survey. This page will also be updated with the number of surveys and subsurveys that you have completed.

Offline Mode

Should you lose internet or data connectivity while completing a survey or using the app, you can continue to complete surveys by saving them as drafts.

When you lose connection, a popup will immediately appear indicating that service has been lost and that the app is now in offline mode. Acknowledge the alert and complete the survey. The app may alert you multiple times that you have lost connection. Simply continue acknowledging the alerts and resume the survey.

If, after completing the survey, you still lack a connection, you can continue to start and complete surveys. The app will continue to save all surveys that you begin and complete in offline mode as drafts.

When connectivity is restored, if you still have drafts saved on your device, you can submit them by going to your accounts page and hitting the “Submit Drafts” button.

*Please note, that in order to protect the confidentiality of clients and their information, contributors can not access surveys after they have been completed, nor access finished drafts after they have been saved.
